Chronic Homelessness in Melbourne: First-Year Outcomes of Journey to Social Inclusion Phase 2 Study Participants

The Journey to Social Inclusion (J2SI) Phase 2 program, delivered by Sacred Heart Mission (SHM), uses an intensive case management approach focused on capacity building and relationship development to facilitate the movement of chronically homeless people in Melbourne to permanent, stable housing and a pathway to broader social inclusion.

The first year of the J2SI Phase 2 program focused on the attainment of permanent housing, stability and addressing immediate health needs. This is measured by the number of participants in permanent housing and emergency department and hospital admissions. The findings cover a broad range of wellbeing domains, including housing, physical health, mental health, alcohol and other drug (AOD) issues, health service utilisation, economic participation, social support and quality of life.
